What it is
In a randomized trial, deep-brain stimulation of the bed nucleus of the stria terminalis and nucleus accumbens produced a 50% response rate and 35% remission across 26 patients with treatment-refractory depression. Combining intracranial recordings with machine learning, the team found that lower BNST theta and top-down prefrontal-BNST coherence predicted better outcomes, held across 3, 6 and 12 months.
Why it matters
Refractory depression has few options, and stimulation targets are usually chosen by anatomy alone. An objective physiological readout of who responds, and why, moves deep-brain stimulation toward closed-loop, biomarker-guided psychiatry.
